Urology Associates, Inc. in Oklahoma City welcomes you to our practice. Once you have made your initial appointment, you may preregister by downloading, printing, and completing the forms below. Doing so prior to your visit will help speed up the new patient registration process.

Your Appointment
Please arrive thirty (30) minutes prior to your appointment time and bring the following:
- Your insurance card(s) and driver’s license
- Your co-pay, if applicable (we accept cash, check, and credit cards)
- A list of current prescription and over-the-counter medications you are taking
- Pertinent information about your medical and surgical history
- Recent X-rays or test results related to your condition
- Completed new patient paperwork, if applicable
If you are unable to make your scheduled appointment time, please call to cancel or reschedule with 24-hour notice to avoid our $35 no-show fee.
At Urology Associates we accept Medicare and most major insurance plans. See the list below. If you do not see your plan on the list, contact your insurance company or you may call our office at (405) 749-9655 to find out if we accept your plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience, we accept cash, checks, Visa, MasterCard, Discover, and American Express.

If you have a question related to your bill or insurance, please contact our billing specialists at (405) 749-9655.
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Urology Associates and they will contact us.
If you have an urgent medical need and need to talk to our doctors after our regular hours, please call our office at (405) 749-9655. A doctor is on-call 24 hours a day. Your call will be returned promptly.
If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit.
Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.
All Physicians at Urology Associates have hospital or surgery center affiliations with:

Some Urology Associates Physicians have additional affiliations. Please contact our office for more information.
Prescriptions and refills for established patients of Urology Associates are routinely issued during our regular office hours (see above). You can make your request by contacting your pharmacy.
No refills are issued during the evenings, weekends, or holidays, as the doctors do not have access to your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
